For permissions please email: [e-mail] report the case of a 44-year-old man who was admitted to our hospital after sudden onset of ventricular tachycardia, hypotension requiring vasopressor administration and documented ST-elevation infarction with corresponding changes in the leads I, aVL, and V1-V3. The patient had no history of cardiovascular disease and no known atherogenic risk factors. Because of the initially documented ECG immediate cardiac catheterization was performed. Coronary angiography revealed the following finding: A thrombus was detected in the left main, which anomalously originated from the right coronary ostium (Panel 1a and b, Supplementary material online, Video S1), without significant atherosclerotic lesions (Panel 1A and C).
